Outpatient/Home Patient Monitoring

Day shift monitoring for step-down patient care.
The Univago HE™ Outpatient/Home Patient Monitoring applications free up patients to receive the care they need outside a hospital setting. This ability to care from afar frees up hospital beds and allows patients to recuperate in a more home-like setting, for example:

  • Patient’s home, e.g. for chronic illness
  • Rehabilitation setting
  • Intensive ambulatory setting
  • Long-term care facility
  • Skilled nursing facility
  • Urgent care clinic
  • Workplace clinic

Part-time monitoring is ideal for patients who need to be overseen after they have been discharged from the hospital and sent home or to another care facility. A tablet or other medical device is used to monitor their vitals and status over time. Video communication enhances the responsible physician’s ability to treat remotely, offering a visual of the patient’s improving or declining condition and interview or assess patient’s compliance with medical instructions.

For inpatients who need to be more closely monitored, Continuous Patient Observation can enable hospital staff to keep a close eye on critical patients while keeping staff-to-patient ratios in balance.

Some settings which can benefit from Continuous Patient Observation include:

  • Hospital ICU
  • Acute care hospital room
  • TeleHospital facility
